Clinical features for Hair loss in Female
Excessive shedding of hair which starts to happen suddenly. Hair starts to shed rapidly and diffusely 2-3 months after some triggering event. Trigger events include severe febrile illness ( e.g- malaria, typhoid, TB etc ), post pregnancy and lactation, major surgeries, physical stress, emotional stress, chronic illness, thyroid disorders, chronic malnutrition and crash diet etc . In about 33% of cases, no trigger event can be identified. Premature termination of anagen into catagen and telogen hair follicle happens in Telogen effluvium.
Female pattern hairloss
Here the hair loss starts gradually and the condition is slowly progressive. Most commonly seen in females aged 20-40 years. It results from a progressive miniaturization of hair follicle due to androgen harmone. Increased androgen levels due to like polycystic ovarian disease (PCOD), and ovarian or adrenal tumors might be the cause. Excessive hair growth on the face, severe acne, menstrual abnormalities, infertility, acanthosis nigricans are few other features associated with increased androgen levels.
Chronic Telogen Effluvium (CTE)
Chronic Telogen Effluvium is characterized by an excessive and diffuse shedding of hairs in females aged 30-60 years, for a duration greater than 6 months. Chunks of hair are seen in the bathroom, pillow, brush, and comb. The trigger factors responsible for acute telogen effluvium (mentioned above) if persists beyond 6 months, then it leads to CTE. In many cases of CTE the underlying cause remains undetectable.
